An Occupational Therapy Evaluation consists of three
subsets:

Developmental History
Sensory Profile

Clinical Observation
Upper Extremity Muscle Strength and Tone
Primitive Reflexes/Protective Reactions
Gross Motor Planning and Performance
Fine Motor Planning and Performance
Body Awareness
Balance and Equilibrium Reactions
Sensory Systems:
Vestibular System
Proprioceptive System
Tactile System
Visual System
Gustatory System
Auditory System
Olfactory System
Engagement
Self-Regulation
•
Meeting with Parents
Collaborating on information pulled from developmental
history, sensory profile, and clinical observations
Reviewing what a generalized home program would look
like
Recommendations for both treatment and outside
referrals, if applicable.
Fees:
Formal Initial Evaluation $500.00
Re-Evaluation $250.00
Looksee (an initial evaluation without a report) is
based off our hourly rate
If a screening has taken place at your child’s school
and a full evaluation is warranted $60.00 which
was paid for your screening will go toward the Formal
Initial Evaluation.
Screenings
• We offer this service on site to specific schools. If
you are interested or would like to get your school
involved feel free to contact us.
Fees:
Handwriting and SI Screenings $60.00 (these are only
offered to specific schools)
